Arizona assisted living owners must meet state licensing and manager certification requirements, including the change-of-ownership process before operating.
Facility licensing and manager certification are separate
The most common misunderstanding among new owners is that these are one requirement. They are not.
The facility holds a licence issued by the Arizona Department of Health Services, through its licensing division for residential facilities. The person managing the home holds a certification issued by the state board that oversees assisted living facility managers. An owner may hold that certification, or may employ someone who does.
Both have to be in place. Satisfying one does not satisfy the other.

What Arizona classifies as an assisted living home
State law defines an assisted living home as a residential facility serving ten or fewer residents. Eleven or more residents falls under a different classification with different requirements.
Licensing is also sub-classified by the level of service provided. Arizona distinguishes supervisory care, personal care and directed care, and the level a home is licensed for determines which residents it can accept. A buyer should confirm the exact level a home is licensed for rather than assuming, because it limits who can live there and therefore what the home can charge.

What a change of ownership involves
When you buy an operating home, the state needs to know who the new licensee is, who will manage the home, and whether they meet its requirements. That process runs on the state’s schedule, not on your closing date.

What the state generally looks at
The exact requirements are set by ADHS and are subject to change, so verify them directly before relying on any of this. Broadly, the application process involves documentation of policies and procedures, evidence of compliance with licensing requirements, residency agreements, resident service plans, and background checks, followed by inspection.
A home’s inspection and compliance history is also worth reviewing during due diligence. Past findings tell you something about how the operation has been run, and about what you may inherit.

Zoning is a separate question from licensing
A state licence does not settle local land use. Cities and counties handle zoning, occupancy, parking, building and fire and life safety requirements for a specific property, and those requirements vary by jurisdiction and by address.

If you are converting a home rather than buying one
Buyers who purchase a vacant house and plan to license it are taking on the licensing process without the cushion of existing revenue. Between conversion work, inspection, licensing and building an initial census, there can be a long stretch with costs and no income.
That is a legitimate path, but it needs working capital planned for it from the start, not discovered halfway through.
